Medical
Physical, dental and vision examinations, prescribed by state law, are required of the following students:
- All children new to the school;
- All children entering grade five in the 2011-2012 school year must have 3 doses of Hepatitis B vaccine and the H.I.B. by the FIRST DAY OF SCHOOL, as required by the Illinois Department of Public Health.
- All children in grades kindergarten, two and six are required to submit a dental and health exam by the first day of school.
- A new law, effective January, 2008 requires that all children enrolling in school for the first time or entering kindergarten shall have an eye exam.
Parents are required to have their children examined by the family physician or a clinic and return all necessary medical information (allergies, asthma, medications, etc.) medical history dental & vision forms to school on or before the first day of school or the child will not be allowed to attend school.
All children entering pre-school or kindergarten in the 2011-2012 school year must show proof of one chicken pox inoculation on or after the child’s first birthday. In lieu of this vaccination, a physician may also confirm past disease history by a physical exam and by documenting the child’s medical history form with laboratory results. A lead screening test by your doctor is required for all incoming preschoolers and new kindergartners.
